Some internship hosts have forms a prospective intern and an official at their college or university are asked to sign. 

Will you be requesting academic credit for the internship?

For most internships, after a student fills out their portion of the form, they should send or take it to the person in charge of the course they would take if they pursue the opportunity for academic credit, usually an academic advisor in their department, but sometimes the college internship coordinator. 

 

Is the internship related to your major? 

Oftentimes departments will advertise openings on their blogs and are aware that the application requires additional forms. You may check with your Academic Advising office to inquire about form signatures.
